Defensible Documentation
Assessment and care planning in the Aged care sector is focused on the principles of person-centered care, working in partnership with older person to develop appropriate care plans and delivering services to meet the individual needs and lived experiences of the older person.
Integral to this is organisational documentation that is accurate and appropriate to the care of the older persons receiving your services.
The way you document in care plans and give you the skills to document the support you provide in a way which is accurate, legally defensible, and improves outcomes for the people you support.
A very important topic that impacts Care and Service Delivery, particularly older persons assessment and development of care plans .

Course outcomes

– Learn the FACT® method, and how to use it to improve your documentation.
– Practice writing clear and concise, defensible progress or case notes.
– Be able to put your new skills into action to document support, change, risks, goals and progress.
– Receive a Quick Reference Guide (QRG) to support ongoing documentation
