Exploring identity, lived experience and culturally responsive care in practice

Care plans are important, but they do not always capture the full story of a person’s life, identity, experiences and what matters most to them.

This CQ Series session will explore how culturally responsive practice goes beyond documentation to better understand the individual behind the care plan. Together, we will reflect on how culture, language, migration experiences, family roles, spirituality, trauma and personal history can shape a person’s preferences, wellbeing and engagement with care.The session will also include perspectives from assessment practice, exploring how meaningful conversations, observation and culturally informed approaches can support more holistic and strengths-based assessment and care planning processes.

Learning Outcomes
By the end of this session, participants will be able to:

  • Recognise and value the diverse lived experiences, identities and strengths older people bring to care relationships
  • Apply culturally responsive and person-centred approaches to build more meaningful conversations during assessment and care planning
  • Identify practical ways to support choice, dignity and connection by understanding what matters most to the individual
  • Reflect on how culture, language, family roles and life experiences can shape a person’s preferences, wellbeing and engagement with care
Target AudienceAll aged care staff including support workers, care staff, coordinators and volunteers working across residential, home care and community care settings.
